With an uptick in need nationwide, evolvedMD will expand its impact to provide upfront and ongoing modern care for over 250 primary care providers across nearly 40 sites in Arizona and Utah by the end of 2021 with growth into Colorado, New York, and New Jersey slated for early 2022.
With this audacious trajectory comes the need for additional committed and passionate people to help support our partners, providers, practices, and patients. We talked before about Boundless Opportunity at evolvedMD and since that note, we’ve increased our team by 58% with new changemakers company-wide including corporate professionals, licensed clinicians, and more.

Michelle Eliseo | Recruitment Specialist
About Michelle: Coming to evolvedMD with nearly 20 years of experience in Human Resources and Recruitment, Michelle prides herself on finding amazing people who are following their superhero calling to change lives. A native Phoenician, she has enjoyed recruiting top talent throughout the Valley in the healthcare space. In her free time, you can find Michelle marathoning true crime podcasts, catching up on her ever-growing stack of books, and kicking back every Friday night for movie night with her family.
Ailise Farrell | Care Coordinator
About Ailise: Living everywhere from Buffalo, Phoenix, Toronto, Ottawa, and Cambridge, United Kingdom, Allie has returned to Arizona to make a meaningful impact in the mental health field. She began her career working at a psychiatric hospital as an intake coordinator after graduating from State University of New York at Buffalo. Now pursuing her Master’s in Clinical Mental Health Counseling with a concentration in childhood and adolescence at Grand Canyon University, she’s excited to take the next step in her career driving positive patient outcomes. In her spare time, she enjoys hanging out with her growing family, bringing awareness to issues affecting Native Americans, and nurturing her love for the outdoors by plant collecting and hiking.
Allison “Allie” Knox | Care Coordinator
About Allie: Born in Ohio and raised in Indiana, Allie escaped the Midwestern winters to warm and sunny Tucson, Arizona, with her family before finally settling in Scottsdale. After graduating from Indiana University in Bloomington and Arizona State University, she worked as a psychometrist and office manager for a small private neuropsychology practice. She is passionate about health and wellness for both herself and others, which makes her a tremendous asset to evolvedMD’s Care Coordination team as they work together to drive improved patient outcomes. When she’s not working, you can find her cooking up a storm at home, playing tennis, and exploring the local restaurant scene with her friends.
Melissa Mann, LAMFT | Behavioral Health Manager
About Melissa: Melissa is a Master’s-level clinician who made the transition into Counseling Psychology after spending 20 years working in law enforcement. A life-long learner with a passion for helping others navigate life, she is thrilled to join the evolvedMD team and make a positive impact on her patients’ lives. In her free time, she enjoys hanging out with her family and is an avid runner who has traveled to Seattle, Las Vegas, Idaho, Disneyland, and many other places to participate in half marathons.
Amparo Medina-Perez | Behavioral Health Manager
About Amparo: Amparo’s career began in law enforcement, where she gained extensive experience working with individuals from diverse backgrounds and traumatic experiences. After learning how nature and nurture potentially influence lifelong outcomes in this field, she transitioned into social work and worked in a clinical setting as a Care Coordinator. Born in Puerto Rico and having lived in New York and Washington state, she eventually found herself in Arizona after falling in love with everything the state has to offer. She is a passionate advocate for basic human rights, social justice, patient care and rights, and LGBTQ rights. During her free time, you can find her traveling the world, discovering new music, meditating for self-care, and playing video games.
Courtney Meola, LAC | Behavioral Health Manager
About Courtney: Courtney kickstarted her career as a licensed counselor working in both University and community mental health settings, where she gained extensive experience treating patients with trauma, anxiety, and depression. Now she finds herself at evolvedMD, ready to drive positive patient outcomes at one of our sites in Phoenix. Born and raised in Arizona, she received her Bachelor’s degree in Psychology from the University of Arizona and her Master’s in Counseling from Arizona State University. She is passionate about empowering others to relieve their own suffering by teaching impactful problem-solving strategies and healthy coping skills to her patients. In her free time, she enjoys working out, tapping into her creative side through painting, and exploring new places throughout the Valley and beyond.
Mary Pals, LPC | Behavioral Health Manager
About Mary: Mary is a compassionate clinician who has held various positions in the mental health field including Mobile Crisis, Case Management for Families First Program, and as a full-time therapist. Before entering the mental health field, she worked in the medical field for 20 years as a Licensed Radiologic Technologist in both staff and management positions. After spending one too many winters in Iowa, she escaped the Midwest to enjoy the year-round warm weather in Arizona. When she’s not working, you’ll find her spending time with family and friends and living it up at the beach any chance she gets.
Evan Sakrison | Strategy & Marketing Specialist
About Evan: Evan is a passionate and creative individual with nearly 5 years of content marketing experience. Possessing a unique background that spans social experimental psychology, entertainment, and artificial intelligence, Evan brings his extensive experience to the behavioral health field as evolvedMD’s Strategy & Marketing Specialist. In this role, he leverages his creative spirit and strategic mindset to drive brand awareness, support growth ambitions, and assist corporate development initiatives. He enjoys hiking, exploring the local music and craft beer scenes, and writing short horror stories in his downtime.
Jade Zinser, LPC | Behavioral Health Manager
About Jade: Jade is a Master’s-level clinician who completed her LPC Master’s program at Southern Methodist University in Dallas, Texas. More specifically, her professional background includes play therapy and most recently neurofeedback to treat a variety of psychiatric conditions. A native Texan, Jade now resides in Phoenix with her fiancé and her golden retriever, Murphy. When she’s not working, Jade enjoys exploring the great outdoors and getting in a good workout.

Humberto Perez | Implementation Specialist
About Humberto: Humberto is a dedicated member of the evolvedMD team who strives to make a positive, long-lasting impact on each patient and provider he interacts with. He has over 7 years of experience in social work and has enjoyed being part of evolvedMD’s Care Coordination team. As an Implementation Specialist, he is responsible for working closely with key staff at practice sites to ensure our Behavioral Health Managers and primary care sites are adequately equipped, both clinically and operationally, to provide the best patient outcomes. Originally from Nogales Sonora, Mexico, he has lived in Arizona since 2010 and graduated from Arizona State University with a Bachelor’s in Criminal Justice and Sociology. In his free time, you’ll find him sweating it out at CrossFit, hanging out with his dogs, and advocating for social justice for a wide variety of causes.
